Move-On & Transition Support

Preparing to move on from supported accommodation is a significant milestone, and we work closely with young people aged 18 to 21 to ensure this transition is positive, safe and well-planned. In cases where a young person turns 21 during their transition, we may continue supporting them for an agreed period if their needs are low and continuity is essential to avoid destabilising their progress. This extension is always planned with the Local Authority and forms part of a structured step-down approach to full independence.

Practical Skills for Independent Living

Our move-on support includes preparing young people for managing a tenancy, setting up utilities, understanding landlord responsibilities, dealing with rent and bills, keeping organised, maintaining routines, and sustaining education, training or employment. We help young adults plan, problem-solve and make informed decisions that will support them long after they leave our service.

Collaboration and Long-Term Stability


We work collaboratively with housing teams, Personal Advisors, Social Workers, colleges and health professionals to ensure that every young person has a sustainable plan in place. Young people are supported emotionally as well as practically so that their transition out of our service feels safe and achievable rather than overwhelming. Our aim is to ensure young adults leave us with confidence, clarity and the skills required to live independently and maintain long-term stability.
